In order to thoroughly implement the national strategy to cope with the aging population, improve the city's elderly care service system that coordinates home community institutions and integrates medical and nursing care, and promote the high-quality development of the elderly care service industry, This plan is formulated in accordance with the spirit of the National Medium and Long Term Plan for Actively Responding to Population Aging, the National Plan for the Development of Aging Undertakings and old-age Service System in the 14th Five-Year Plan, the Special Plan for the Old-age Service System in Shaanxi Province, and the Plan for the Development of Old-age Undertakings in Xi 'an in the 14th Five-Year Plan.


First, the development background


1. Major achievements of the 13th Five-Year Plan Period


 The development of the city's elderly care service industry has achieved certain results.


The elderly care service system has been continuously improved. Improve the relevant laws and regulations on elderly care services, and take the lead in the province to issue the "Xi 'an Elderly Care Service Promotion Regulations" local regulations, through legislation to promote, promote and standardize the development of elderly care services in our city. The Implementation Opinions of Xi 'an People's Government on Accelerating the Development of the Elderly Care Service Industry, the Layout Plan of Xi 'an Elderly Care Service Facilities (2018-2030), and the Implementation Rules for Star Rating and Operation Rewards of Xi 'an Elderly Care Institutions have been issued, and the policy system of elderly care service has been continuously improved and improved.


The supply of elderly care services continues to expand. During the "13th Five-Year Plan" period, the city has built 2,406 various kinds of elderly care service facilities, including 168 elderly care institutions, 805 community elderly care service stations, 1,280 rural mutual assistance and happiness homes, 121 day care centers, and 32 town and street comprehensive elderly care service centers, with the coverage rate of urban and rural community elderly care facilities reaching 80% and 70% respectively. A system of elderly care services based on the home, supported by the community, supplemented by institutions, and combining medical and nursing care has been established.


Safety guarantees for elderly people have been strengthened. We will provide care subsidies to the guardians of scattered dependents in extreme poverty, care subsidies to the disabled elderly, living allowances to the disabled and poor elderly aged 65 to 69 in rural areas, and additional subsistence allowances to the elderly over 70 in families receiving subsistence allowances. Accident insurance has been purchased for elderly people with special difficulties, such as those aged 80 and above, and a relatively complete insurance system for old-age services has been established.


The quality of elderly care services has been continuously improved. For three consecutive years in the city to carry out the "nursing home service quality construction special action", in-depth investigation and rectification, eliminate the rectification of fire, food and special equipment and other safety hazards 138. The first in the province to carry out the work of star rating of pension institutions, three star and above pension institutions accounted for more than 50%. Since 2017, skills training for elderly care workers and managers has been carried out every year to gradually improve the level of talent construction in elderly care institutions. The elderly care service market continues to develop. During the "13th Five-Year Plan" period, we actively mobilized social forces to participate in the development of elderly care services, and successively introduced a series of incentive subsidies, tax incentives, and fee reduction policies to support the development of the elderly care industry. For-profit and non-profit elderly care service institutions enjoy the same subsidy treatment. The one-time construction subsidy and operation subsidy standards for social capital investment in the construction of elderly care institutions were greatly increased, and the capital investment in elderly care services was increased, with private institutions accounting for 78% of the city's elderly care institutions.


(2) Development situation during the 14th Five-Year Plan Period


The "14th Five-Year Plan" period is a key window period for actively responding to the aging population and promoting the development of elderly care services. While grasping a series of important opportunities, it is also facing severe challenges.


Development opportunities. With the continuous improvement of living and medical standards, the life expectancy of the population continues to extend, and the aging of the population is rising rapidly. According to the seventh national census, as of 2020, Xi 'an has 12.9529 million permanent residents, and 2.0753 million people aged 60 and above, accounting for 16.02%. It is expected that by 2025, the permanent resident population of Xi 'an will reach 15 million, of which the population aged 60 and above will reach 3 million, accounting for more than 20% of the total population, and will enter the moderate aging stage from the mild aging stage. In the coming period, Xi 'an will face the problem of rapid increase in demand for elderly care services and relatively insufficient service supply. Speeding up the high-quality development of elderly care services is an important measure to ensure and improve people's livelihood, and is of great significance to achieve elderly care and better meet the people's ever-growing needs for a better life.


Face challenges. During the "14th Five-Year Plan" period, Xi 'an entered a period of rapid development with an aging population. While the elderly care service was transforming to high-quality development, it also faced problems such as poor working mechanism of elderly care service, imperfect basic elderly care service system, mismatch between supply and demand of elderly care service, slow development of rural elderly care, inadequate combination of medical and nursing care, imperfect supervision mechanism and inadequate element guarantee.


Second, general requirements


(1) Guiding ideology


 We will develop inclusive services for the elderly, promote mutual assistance services for the elderly, support families in assuming the functions of the elderly, foster new forms of services for the elderly, promote the coordinated development of the elderly service and the elderly care industry, actively build a service system for the elderly that coordinates home community institutions and integrates medical care and nursing care, and continue to enhance the sense of gain, happiness and security of the elderly.


(2) Basic principles


-- Government-led and multi-party participation. Adhere to the leadership of the Party committee, government leadership, social participation, and action by the whole people, and build a basic old-age care pattern in which the government, society, families, and individuals participate together and each performs his or her responsibilities. Consolidate the basic role of family care for the elderly, fully mobilize market forces, cultivate professional and large-scale providers of elderly care services, and build an age-friendly city that is compatible with the construction of national central cities.


-- People-oriented, demand-oriented. Adhere to the people-oriented elderly care service concept, based on solving the problems of "urgent", "difficult", "worry" and "hope" for the elderly, deepen the connotation of elderly care services, improve the talent system of elderly care services, further promote the combination of medical and nursing care, promote the improvement of care quality, form a pension service system that meets diverse needs, and let the elderly share the results of reform and opening up.


-- Ensuring basic security and promoting inclusive development. We will strengthen the government's role in providing basic and bottom-line services for the elderly to ensure the most basic needs of the elderly. Actively support the development of inclusive elderly care services, provide affordable, safe, reliable, convenient and accessible elderly care services for the elderly, and meet the personalized, diversified and multi-level needs of the elderly group.


-- Reform and innovation and coordinated development. We will create new service models for the elderly, foster new industries and forms of business for the elderly, promote deep integration of the elderly with education and training, health, sports, culture, tourism, home economics, and rehabilitation AIDS, and promote coordinated development of the elderly service and the elderly care industry.


3. Development goals


By the end of 2025, the basic elderly care service system for everyone will be further established, the network of elderly care service facilities will be improved, the supply structure of elderly care services will be more optimized, industry elements will be supported more strongly, comprehensive supervision will be scientific and effective, and the informatization, standardization and specialization of elderly care services will continue to improve. A service system for the elderly featuring coordination between home and community institutions and a combination of medical and nursing care, and effective coordination and high-quality development of elderly care services and industries, including the following five aspects:


1. Basic old-age service system has been established. We will establish a list of basic elderly care services, fully support those in extreme poverty, fully implement a comprehensive assessment system for the elderly, provide home visits to elderly people with special difficulties, improve a tiered assistance system for the elderly, and explore the establishment of a long-term care insurance system.


2. The facility network is more sound. According to the layout idea of "one community, one station, one town street, one center, one district and county, one hospital", each district and county has built a public demonstration nursing home and put it into operation, and the coverage rate of comprehensive elderly care service centers in towns and streets has reached 70%, and the coverage rate of community elderly care service facilities has reached 90%.


3. Better service supply. The level of smart elderly care services has been effectively improved, a system of meal assistance services for the elderly has been basically established, the work of upgrading homes suitable for aging has been carried out in an orderly manner, pilot projects for elderly care beds in families have been further expanded, and mutual assistance for elderly care in rural areas has continued to develop. Deep integration of medical and nursing care, nursing beds in the city's elderly care institutions accounted for 60%.


4. Factor support continues to be enhanced. We have basically made up for the shortcomings of land for the elderly, established a sound mechanism for diversified investment in elderly care services, basically met the standards for the allocation of elderly care service facilities in new residential communities, effectively strengthened the construction of senior care service personnel, and made the supporting role of the factor guarantee system more obvious.


5. Increasing regulation. The comprehensive supervision system for elderly care services has been improved, the supervision work of "double random and one open" of elderly care institutions has been effectively promoted, the efficiency of supervision has been significantly improved, and the comprehensive supervision has been more legalized, standardized and normalized, so as to achieve full coverage, all-round and whole-process supervision of the activities of regulated objects.

Third, main tasks


1. Establishing and improving a basic old-age service system


1. Establish a basic elderly care service list system. We will formulate and publish a list of basic elderly care services, clarify the service target, service content, service standards and expenditure responsibilities, and prioritize elderly people with financial difficulties, disability, severe disability, empty nest, left-behind, and special family planning families as key targets. According to the economic and social development level of our city and the aging population situation, we provide suitable services for different elderly groups such as old-age security, life care, rehabilitation care and social assistance. By 2025, the list of basic elderly care services will continue to be improved, and the mechanisms for service supply, service guarantee and service supervision will continue to be improved, and basic elderly care services will be accessible to everyone.


2. Give full play to the role of supporting service facilities for the destitute. We will strengthen the upgrading of support service facilities for the extremely poor to adapt to aging conditions, promote the upgrading of fire protection facilities, and enhance the capacity of care services for disabled elderly people. We will provide centralized support for the elderly in extreme poverty who are willing to provide centralized support, so that they are willing to provide full support and should provide full support. On the premise of meeting the needs of the extremely poor to provide centralized support, the remaining beds will be allowed to open to the public, the waiting list pilot will be promoted, and a scientific and reasonable charging and price formation mechanism for public elderly care institutions will be established. We will promote the reform of the salary and treatment of employees of poverty-stricken support service institutions, and raise the salary level of elderly care workers in bottom-line security institutions.


3. Improve the comprehensive assessment system for the elderly. We should promote the ability assessment standards for the elderly, and carry out practical operation training and application guidance. We will carry out comprehensive assessment of the abilities of the elderly, provide comprehensive assessment of the abilities of the elderly over the age of 65 according to their personal wishes, and include assessment costs for the elderly in extreme poverty, subsistence allowances, and low-income households in the annual budget. We will foster institutions and teams for comprehensive assessment of the abilities of the elderly, and strengthen capacity building and standardized management. Relying on the comprehensive information platform for elderly care services, an information management system for elderly people's ability assessment is established to supervise and manage the assessment work. By 2025, the comprehensive ability assessment system for the elderly will be basically established, and the assessment results will serve as an important basis for the elderly to enjoy relevant services and subsidies.


4. Improve the welfare subsidy system for the elderly. We will implement a nursing subsidy system for people in extreme poverty, and effectively improve the level of security for people in extreme poverty. Improve the nursing service subsidy system for disabled elderly people with difficulties in life, and ease the care burden of disabled elderly people with difficulties in life. We will improve the system of living allowances for rural disabled and poor elderly people, and simplify the procedures for reporting, approving and granting them. Subsidies for living and health care will be provided to elderly people over the age of 70, and preferential services for the elderly will be provided. We will continue to carry out the work of accident insurance for the elderly, and purchase accident insurance for the extremely poor dependents of home care, key entitled groups, and the elderly over the age of 80, so as to improve the ability of the elderly and their families to resist risks.


5. Strengthen visiting care services for the elderly. Civil affairs departments at all levels guide towns (streets) to carry out investigations of elderly people with special difficulties such as empty nests, left-behind, disabled, severely disabled, and family planning special families, and establish a basic database relying on a comprehensive information platform for elderly care services. Give full play to the professional advantages of elderly care service institutions, take the government to purchase services, and carry out visiting care services through the community elderly care service station and other service facilities network. Enrich the content of visiting care services, comprehensively understand the health, family, economy, safety and other conditions of the visitors, and do a good job in emergency handling in special periods. By the end of 2023, we will basically establish a care and care service mechanism for elderly people with special difficulties, and all districts and counties will introduce implementation plans in light of their actual conditions. By the end of 2024, the visiting care service will be effectively implemented. By the end of 2025, we will ensure that the monthly visit rate of elderly people with special difficulties reaches 100%, that elderly people with difficulties can receive effective help, and that the care and service mechanism is more sound.


6. Actively promote the establishment of a multi-level long-term care security system. We will promote the establishment and improvement of a long-term care security system for the elderly that integrates social assistance, social welfare and social insurance. Promote according to the national long-term care insurance system pilot requirements, explore the design of long-term care insurance system in our city, and actively apply for long-term care insurance pilot cities. We will give full play to the complementary role of social assistance, social welfare, charity and commercial insurance to ensure the needs of care at different levels. Regulate and guide commercial banks, insurance companies, securities companies and other financial institutions to develop finance, credit, insurance and other financial products suitable for the elderly.


(2) Promoting the coordinated development of home-based community institutions


7. Improve the network of service facilities for the elderly. Establish and improve a three-level network of elderly care service facilities in districts (counties), streets (towns) and communities (villages) covering urban and rural areas with balanced distribution and complete functions. At the end of the "14th Five-Year Plan", at the district (county) level, at least one district and county demonstration public nursing home with professional care for the disabled elderly has been built, and the number of beds has reached 1% of the elderly population in the district. At the street (town) level, the construction of street (town) comprehensive elderly care service centers with full care, day care, door-to-door service and other functions, to achieve 70% coverage. At the community (village) level, the development of community elderly care services, rural happiness homes and other facilities, to achieve community elderly care service facilities coverage of 90%.


8. Improve family support policies for elderly care. Adult children are encouraged to live nearby or together with their elderly parents, fulfill their maintenance obligations and take care of their elderly parents, and implement a system of paid nursing leave for children. We will promote the inclusion of training on the care of disabled, mentally retarded and elderly family members in the catalogue of elderly care services purchased by the government, and enhance the capacity of family care. Elderly, disabled and disabled elderly families that are included in the dispersed support for the poorest people, people living out of poverty, recipients of subsistence allowances, and low-income families will be reformed to adapt to aging conditions and given certain subsidies. By the end of the "Fourteenth Five-Year Plan", 7,000 families will have completed the renovation of their homes for aging.


9. Enrich the functions of home-based community elderly care services. Encourage disabled elderly people as the focus, give full play to the supporting role of medical and nursing care institutions, vigorously promote the elderly care service facilities in the community (village) of the street (town) integrated elderly care service center chain operation area, promote the extension of professional elderly care services to the community (village) and families, and create a "15-minute elderly care service circle". We will continue to carry out the pilot project of family elderly care beds, and introduce professional elderly care service institutions to provide long-term care services for elderly people at home. Encourage and guide state-owned enterprises, social organizations, medical institutions, domestic service companies, property management enterprises, etc. to participate in home-based community elderly care services, and provide various forms of elderly care services in accordance with the principles of convenient proximity, safety and quality, and reasonable prices. Actively promote the construction of elderly assisted meals in urban and rural communities (villages), guide market players to develop catering products and establish distribution systems for the elderly.


10. Continue to optimize the service supply of elderly care institutions. Focus on the rigid demand for long-term care services for the elderly and disabled elderly, make good use of existing elderly care facilities, improve the care capacity of elderly care institutions, and strive to increase the proportion of nursing beds. Further promote the star rating of elderly care institutions, and promote the high-quality construction, scientific and standardized operation, and service level improvement of elderly care institutions. By 2025, the proportion of nursing beds in elderly care institutions will reach 60%; The proportion of elderly care institutions above three stars has reached 60%, and the town-level public elderly care institutions rated as one to two star service levels and the district and county public elderly care institutions rated as two to three star service levels have reached 80%.


11. Support the development of inclusive services for the elderly. Encourage all levels to formulate supportive "policy packages", drive enterprises to provide inclusive "service packages", and promote the construction of a number of convenient and accessible, affordable, quality guaranteed elderly care service institutions. Guide various subjects to provide inclusive elderly care services, expand service supply, improve service quality, improve the sustainable development mechanism, and further improve the inclusive price formation mechanism under the market principle, elderly care service institutions that enjoy government subsidies and free or low compensation to use site facilities should be inclusive oriented to determine service prices and open to the public.


(3) Strengthening the effective supply of elderly care services in rural areas


12. Promote the construction of rural elderly care infrastructure. In response to the worsening aging of the rural population, rural elderly care services will be included in the strategic layout of rural revitalization. Carry out research on the construction of rural elderly care service system, and comprehensively find out the development status of rural elderly care service facilities. Taking into account factors such as regional population size, distribution of the elderly, demand for elderly care services and sustainable development of institutions, the layout of service facilities should be adjusted from a regional perspective. In accordance with the idea of "classification and functional complementarity", regional comprehensive elderly care service centers with comprehensive service functions and the combination of medical and nursing care will be built, and service facilities of rural mutual assistance and happiness homes will be upgraded and reformed, so as to form a rural basic elderly care service network with support services for the extremely poor, regional comprehensive elderly care services and mutual assistance services.


13. Promote the continuous and standardized operation of rural mutual aid and happiness homes. Continue to improve the support policies of rural mutual assistance and happiness homes, encourage social organizations, elderly associations, and elderly service institutions to participate in the operation and management of rural mutual assistance and happiness homes, support social forces to focus on the rigid needs of rural elderly people, and carry out elderly care services such as elderly meals, rehabilitation nursing, empty nest care, and cultural entertainment. Focus on supporting districts and counties as regions, contiguous management, chain operation, the formation of branded, chain service institutions. Encourage all districts and counties to explore and form typical cases of the operation mode of rural mutual assistance and happiness hospitals according to local conditions, and promote and apply them in the whole city in a timely manner. By the end of the "14th Five-Year Plan", the continuous standardized operation rate of the rural mutual aid happiness hospital has significantly improved.


14. Explore the development of rural mutual assistance for the elderly service model. We will support grassroots autonomous organizations, social work stations and grassroots elderly associations in carrying out mutual assistance activities for the elderly, and improve the social support network for the elderly. Cultivate rural mutual assistance service teams, widely carry out volunteer services for the elderly, support the establishment of mutual assistance or volunteer service teams with rural women and younger elderly people as the main body, uphold the concept of "helping others and helping themselves", and provide volunteer services for the elderly and disabled. Improve the rural community care inspection mechanism, strengthen the accident risk prevention of rural left-behind elderly people.


4. Promoting the integrated development of medical and nursing care


15. Increase the supply of integrated medical and nursing services. Encourage and support qualified elderly care institutions to apply for or cooperate with public hospitals to set up various medical and health institutions, such as rehabilitation hospitals, nursing homes, and traditional Chinese medicine hospitals, to provide the disabled and mentally retarded elderly with needed medical care and life care services. The medical and health institutions in the elderly care institutions shall be included in the management of the medical union, and the medical insurance quota of the medical institutions organized by the elderly care institutions shall be reasonably verified. Eligible elderly care institutions transformed from medical institutions shall enjoy corresponding subsidy policies according to regulations.


16. Establish and improve cooperation mechanisms for elderly care and medical and health services. Promote medical and health institutions to strengthen the basic medical service capacity of elderly care institutions and home-based community elderly care service facilities by signing services, agreement cooperation, opening green channels for medical treatment and other forms. Encourage medical and health institutions to apply for old-age care institutions, support public hospitals, state-owned enterprises and social forces to set up old-age care institutions that combine medical and old-age care, revitalize existing medical and old-age care resources, and achieve two-way integration and interoperability of medical and old-age care services. By 2025, elderly care institutions will generally have the ability to combine medical care with nursing care.


17. Improve the level of health care integration. Give full play to Xi 'an's rich leisure tourism resources, focus on the "tourism, cultural support", the development and construction of a number of health bases. Promote the construction of sojourn demonstration counties (cities and districts) and institutions (bases), build cultural tourism projects for the elderly, and build interactive sojourn alliances. Fully explore the characteristics and advantages of traditional Chinese medicine in our city, and develop traditional Chinese medicine health care services. We will promote cooperation between TCM hospitals, community health service centers, township health centers, elderly care institutions, and community elderly care service facilities, and widely promote appropriate TCM technical services such as massage, cupping, and moxibustion. Support elderly care institutions in setting up TCM clinics to provide elderly health care services with TCM characteristics.


(5) Accelerate the development of the elderly care service industry


18. Optimize the market environment for elderly care services. We will fully liberalize the market for elderly care services, cancel permits for the establishment of elderly care institutions, and manage registration and filing in accordance with the law. According to the development status of the city's elderly care services, it has continuously revised and improved various elderly care service policies, continuously issued a list of preferential support policies in the field of elderly care services, and implemented detailed tax incentives, fee reductions, and preferential pricing policies for residents such as electricity, water and gas to support the development of elderly care services. Support social forces to participate in the operation of elderly care service institutions, promote the chain, branding, and scale construction of elderly care service institutions, and appropriately give certain rewards. Continue to hold the elderly service industry expo, cooperation and exchange meetings, do a good job in the link and integration of high-quality elderly service resources, and improve the visibility of elderly service in our city.


19. Actively foster a silver economy. Vigorously promote the diversified integration of "elderly care services + industries", and support the integration and development of elderly care services with culture, catering, housekeeping, education and other industries. Promote the standardized development of food, drugs and elderly products industries closely related to the lives of the elderly, support enterprises and scientific research institutions to increase the research and development and manufacturing of elderly products, improve the function and quality of traditional elderly care products, and meet the special needs of the elderly. Foster the rehabilitation aid rental market, promote the community rental services and application of rehabilitation AIDS, and promote the research and development and production of various rehabilitation AIDS suitable for elderly care service institutions and families.


20. Improve the level of smart elderly care services. Promote the development of "Internet + elderly care services", and comprehensively promote the municipal comprehensive information platform for elderly care services. Strengthen the business collaboration and data linkage of various information systems such as government, enterprises, and society, extend the platform to districts (counties), streets (towns), communities (villages), and institutions, and continue to build a digital elderly care service system integrating "service + management + supervision + decision-making". Support elderly care service institutions through the platform display, take the "children online order, the elderly experience service" model, accurate docking for the elderly service needs, to provide "menu" nearby and convenient service for the elderly. Encourage elderly care service enterprises to innovate service models, customize health monitoring systems for various activity scenarios for the elderly, and use big data to facilitate home travel, health management and emergency response for the elderly. Actively organize elderly care service institutions to declare smart health elderly care service products and service promotion catalogs.


21. Promote cross-regional cooperation in elderly care services. Strengthen cross-regional docking and linkage, and promote the circulation of elderly care service elements, project cooperation, and elderly care industry linkage among cities. Strengthen the integration process with Weinan, Xianyang and other cities, and carry out multi-dimensional cooperation such as purchasing services, combination of medical care, talent training, and institutional pension. Continue to carry out cooperation and exchange of talents and institutions between Xiwei, Xihan, Xixian and other regions, build platforms for exchanges and mobility of professional talents between regions, and provide better elderly care services for the masses.


(6) Improve the level of orderly development of industry norms


22. Promote the standardization of elderly care services. Give full play to the role of the Standardization Committee of elderly care services, deeply implement industry guidelines such as the Basic Standards for the Service Quality of Elderly care Institutions, the Classification and Evaluation of elderly care Institutions, and the Basic Standards for the service safety of elderly care Institutions, and guide social organizations, enterprises and scientific research institutions in the elderly care service industry to formulate and issue standards to improve the overall development level of the industry. Strengthen the standardization publicity in the field of elderly care services, and enhance the consciousness and enthusiasm of formulating standards, using standards, and learning standards in the field of elderly care services.


23. Comprehensively strengthen comprehensive supervision of elderly care services. We will improve the filing management system for elderly care institutions, implement classified management of elderly care institutions, distinguish different fields, forms, types and functions of elderly care services, and gradually establish a full-coverage, all-directional and whole-process supervision pattern.
